Withdrawal form
The model form for withdrawing from a distance contract. You do not have to use it — any clear statement will do — but it is here if you want one.

1When this applies
Clause 8 of the competition terms explains what you can get back and when. In short: slots you have not used are refundable before uploads open; a slot on which you have already uploaded a work is not, because judging preparation has started on it.
You do not need this form. An email saying you have changed your mind, with your order number, has exactly the same effect.
2The form
Copy the lines below into an email to contact@expert-lash.com, or print and post them to Str. Anton Pann nr. 14-16, Corp B, parter, Cluj-Napoca, jud. Cluj.

Cluj, contact@expert-lash.com: I hereby give notice that I withdraw from my contract for the following entry slots — ORDER NUMBER — ordered on DATE — name of consumer — address of consumer — signature (only if this form is sent on paper) — date.
3What happens next
We confirm receipt, and we refund within 14 days of being told, to the card you paid with. We do not ask you to accept a voucher instead.
If part of your order can no longer be withdrawn — a slot you have already used — we say so, with the reason, and refund the rest.
